AUTHORIZATION TO PICK-UP

For the safety of our children, we must be notified, preferably in writing, if someone other than the parent or regular caregiver will be picking up your child. They will then be required to provide picture ID upon arrival. The following people - other than those listed above - are authorized to pick up my child:

As parent and/or guardian, I do authorize the treatment of the following minor(s) by qualified and licensed medical doctor(s), in the event of a medical emergency, which, in the opinion of the attending physician, may endanger the child’s life, cause disfigurement, physical impairment, or undue discomfort if delayed. This authority is granted only after a reasonable effort has been made to reach me.

Tuition Payment Plans and Policy:

Wings Preschool is an annual tuition, that is broken down into a 12 installment payment plan for your convenience. The 1st Installment is due at registration. Please confirm your understanding that this payment is non-refundable and non-transferable. We cannot make exceptions to this policy. You may pay with cash/check in person writing: 25-26 School year in the memo/on envelope. An invoice will be emailed to you shortly after registration with a pay link for those wishing to pay by credit card. If no payment is received within 90 days of an invoice, all payment is due prior to your child being able to return to class. A $150 cancellation fee will be charged for those families that start the school year in September and withdraw before June. Please sign and date to indicate that you understand our payment plan and policy.
